Intake
The problem
Patients filled in paper forms at reception, staff re-typed them, and referrals arrived by fax. No-shows ran high because reminders went out by phone call during working hours.
The approach
- Intake moved to the patient's own phone before the visit, in English or French, with the health card captured by camera.
- A staff console shows arrivals, missing fields and referrals in one list, replacing three paper trays.
- Reminders go by SMS with a one-tap confirm, and the schedule reflects confirmations in real time.
- Accessibility was tested with older patients and screen-reader users, meeting WCAG 2.2 AA and AODA.
What ships
Reception time per patient cut to a check of the screen, bilingual forms completed before arrival, and reminders that patients actually answer.
